SOCCER SCHOOL REGISTRATION

The cost of any personal soccer gear is the responsibility of the student and their family. Shin pads are highly recommended and indoor soccer shoes will help protect your child.

Each session will have 16 individual sessions at a cost of $225 for St. Emile students.

Regular program price is $250.00 (please add $50 for uniform if needed).

Please include payment of cash or cheque addressed to UDM Union Deportiva de Manitoba with this form.

Waiver & Release of Liability

I understand that there are risks associated with my participation in this Activity, such as physical and/or psychological injury.

In consideration of UDM Soccer School allowing me to participate, I agree:

a) To assume all risks arising out of, associated with or related to my participation;

b) To be solely responsible for any injury, loss or damage that I might sustain while participating; and

c) To release the Organization from liability for any and all claims, demands, actions and costs that might arise out of my participation, even though such risks, injuries, loss, damage, claims, demands, actions or costs may have been
caused by the negligence of the Organization.

I acknowledge that I have read this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, executors, administrators and representatives.
